The referral and transition process is difficult to navigate, resulting in poor outcomes and inappropriate or unnecessary utilization of services. This Power Pack describes how practices can coordinate care in ways that lead to improved engagement, efficient patient flow, and better health outcomes.

Many practices receive conflicting information on clinical best-practices. This Power Pack describes how practices are using clinical guidelines to implement evidence-based approaches to care.

The Transforming Clinical Practice Initiative (TCPI),  developed by the Centers for Medicare & Medicaid Services (CMS), assists 140,000 clinicians in changing the way they deliver health care through technical assistance to implement process and quality improvements. TCPI supports primary and specialty clinicians, nurse practitioners, physician assistants, clinical pharmacists, and practices to 

TCPi established Support and Alignment Networks (SAN) to spread practice transformation to clinicians, practices, and partner organizations. The Power Packs were developed by the 10 SANs.
